Why a Common Certification Application?

The Columbus Regional Certification Application is a joint effort of the:

The common certification application form has been developed as a result of the desire of these organizations/agencies, each with minority, women and/or disadvantaged business certification programs, to reduce the time business owners must devote to completing the certification process.

Who should complete this form?

If you are a minority, disadvantaged or women owned enterprise interested in obtaining certification with any of the above noted organizations/agencies, you should complete this form.

The common certification application can be completed only by those who are seeking a new certification; it is not intended for those seeking re-certification. If you want to apply for re-certification, please contact the appropriate certifying agency.

What happens after I complete the form?

Your application will be submitted to those organizations/agencies that you designate. The organization/ agency will notify you that they have received your application. Please note that in addition to the application, supporting documentation is required by the organizations/agencies and some organizations/agencies may require a fee. Download Instructions.

An on-site visit is required to complete the certification process.

Each organization/agency retains the right to certify or deny certification. The organization/agency shall notify the applicant of the decision reached relative to the request for certification.
