Why a Common Certification Application?
The Columbus Regional Certification Application is a joint effort of the:
- Central Ohio Transit Authority
- City of Columbus, Equal Business Opportunity Commission Office
- Columbus Regional Airport Authority
- Ohio Department of Administrative Services, Equal Opportunity Division
- Ohio Department of Transportation (Disadvantaged Business Enterprise Program)
- South Central Ohio Minority Business Council
Who should complete this form?
If you are a minority, disadvantaged or women owned enterprise interested in obtaining certification with any of the above noted organizations/agencies, you should complete this form.
The common certification application can be completed only by those who are seeking a new certification; it is not intended for those seeking re-certification. If you want to apply for re-certification, please contact the appropriate certifying agency.
What happens after I complete the form?
Your application will be submitted to those organizations/agencies that you designate. The organization/ agency will notify you that they have received your application. Please note that in addition to the application, supporting documentation is required by the organizations/agencies and some organizations/agencies may require a fee. Download Instructions.
An on-site visit is required to complete the certification process.
Each organization/agency retains the right to certify or deny certification. The organization/agency shall notify the applicant of the decision reached relative to the request for certification.
